Terms and Conditions:
After the successful payment, client will receive a confirmation email from Furmomo and client will be contacted by Amazing Grace Animal Hospital & Wellness Centre within 2 working day for the confirmation of booking date and location. Client can also query your transaction in the next working day. (General Enquiry Hotline: 2711 8482)

Validity
The health check package is valid for six months (from the date of purchase) or it will be forfeited.

Remarks:
1. Client must present order confirmation email on the appointment day.
2. The purchase cannot be altered or refund after the confirmation of order.
3. All health screening test or examination is for the purpose of medical diagnosis or treatment.
4. You cannot specify a veterinarian when using this promotion.
5. If a urine test is included, you can inquire with the staff at the time of booking. They can pre-arrange for you to pick up a urine container at the clinic, so you can prepare by collecting a urine sample on the day of the test for analysis.
6. Furmomo and Amazing Grace Animal Hospital & Wellness Centre reserve the right of reserve the right of final interpretation.
